Abstract |
Even though non-ketotic hyperglycemia is a metabolic disorder, it rarely complicates chorea and ballismus. We describe the case
of an 85-year-old man with a sudden onset of left-sided hemichorea with extrusion of the tongue. Laboratory screen revealed uncontrolled diabetes with absent ketones and normal serum osmolarity. Radiologic image showed abnormal lesions in the right basal ganglia. The involuntary movements disappeared within three days of blood glucose control. Follow-up CT revealed gradual resolution of the abnormal lesions in the basal ganglia. The resolution of this patient's hemichorea with glycemic control, as well as characteristic imaging findings, support the diagnosis of non-ketotic hyperglycemia-induced chorea. When properly identified and treated, the condition has an excellent prognosis. |
